Excessive underarm sweating (primary axillary hyperhidrosis) affects a population that many times suffer in silence. An underdiagnosed and underreported issue, excessive underarm sweating affects almost 8 million people in the United States. That is 3% of the US population which is nearly the population of North Carolina. This condition affects sufferer's occupation, emotions, psyche, and can even cause physical impairment. There is a list of options ranging from non-invasive temporary solutions to a permanent cure for this condition.

Excessive underarm sweating affects afflicted individual's ability to perform in the workplace, be comfortable in public, meet people, and develop personal relationships. Many clients report changing their clothing several times per day and editing colors they choose to wear. Individuals say light or bright colors such as light gray or khaki show sweat badly, and they do not wear them. Silks and satins show sweat more readily, and sufferers avoid these materials as well. One study reported that over one-half of clients were moderately or severely affected emotionally by their excessive underarm sweating.

The severity and impact of excessive underarm sweating are assessed by the Hyperhidrosis Disease Severity Scale (HDSS) ranking. The HDSS scale rates the impact of sweating on an individual's daily activities:

  1. Never noticeable, never interferes
  2. Intolerable, sometimes interferes
  3. Barely tolerable, frequently interferes
  4. Intolerable and always interferes
The score corresponds to the statement most pertinent to the client's experience. Most clients experiencing severe impact from this condition score a 3 or 4. This questionnaire is an easy and valid tool for screening clients for the condition, but the bottom line is: Does excessive sweating bother YOU!

Many treatments exist to manage excessive underarm sweating. Topical agents, oral medications, and Botox or Xeomin injections are available as temporary treatments for excessive underarm sweating. Surgical solutions, such as removal of underarm sweat glands subcutaneous liposuction, and endoscopic thoracic sympathectomy (ETS), are permanent solutions but have many side effects. Since January 2011, microwave technology is now FDA cleared for the treatment of excessive underarm sweating called miraDry.

This technology is safe and a permanent solution for treating excessive underarm sweating in one or rarely two treatments with no long-term side effects. Few clients are not candidates for miraDry. Those clients include people with heart pacemakers and other electronic device implants, clients needing supplemental oxygen during treatment, and clients with known resistance to or history of intolerance of local anesthesia including lidocaine and epinephrine. There are no long term side effects with miraDry and bromhidrosis (excessive underarm odor) is treated permanently simultaneously.
